New Process for Obtaining a Patient Card in Nevada
In August 2016, the Medical Marijuana Division of the Nevada Division of Public and Behavioral Health changed the process for patients to obtain medical marijuana cards. There is no longer any need to visit the DMV for patient cards. To apply and pay for a medical marijuana card, patients now use the Medical Marijuana Program’s (MMP) online system.
To use the MMP system, go to: http://dpbh.nv.gov. Click on “Medical Marijuana Dispensary” and then “Medical Marijuana Patient Card Registry.” From there, click “Online Cardholder Registry” to create an account, scan your driver’s license, download and print your application. As before, to submit an application, you still need a physician’s recommendation and you’ll need to sign the waiver and acknowledgement in the presence of a Notary Public. Once your application is complete and signed, you scan and upload it to the MMP site. Letters of approval may be received in as little as 24 hours and can be printed and used in dispensaries until your patient card arrives in the mail.
Note: Patients who have not picked up their card but have approval letters dated June 1, 2016 or later will receive their card in the mail. Patients who have not picked up their card but have approval letters dated May 31, 2016 or earlier will be contacted by the MMP.
Print and Mail Patient Cardholder Application
If you prefer to print, fill out and mail your medical marijuana patient application, you can do so. Visit http://dpbh.nv.gov. Click on “Medical Marijuana” and then “Medical Marijuana Patient Card Registry.” From there, click on “Forms” in the right hand column and then “Application Request Form” and follow the instructions.
Change of Address
To change your address, visit http://dpbh.nv.gov. Click on “Medical Marijuana” and then “Medical Marijuana Patient Card Registry.” From there, click on “Forms” in the right hand column, then “Change of Address Form” and follow the instructions.
To request an application for the Medical Marijuana Program — $25
To register or renew registration in the Medical Marijuana Program — $75
There is no fee to change your address.
